Incident reporting allows Staff to record incidents as they happen and gives Managers updates and insights into any necessary follow-up actions.
Note: Different Departments can have different Incident Reports. These are created by customising an Incident Report Template. For more information, read this article (Creating and Customising Incident Report Templates).
Reporting incidents in Clipboard will be determined by what Activities they are assigned to. All users can report an incident in Clipboard that isn't directly linked to a Session in the Schedule.
However, Staff and Manager users can only report an Incident for an Activity or Session that they are assigned to.
To report an incident:
Go to Incidents
Click Report an Incident (top right corner)
An Incident Form will pop up. Fill in the form including details about the student/s involved, the type of incident and whether any further action is required.
When you're done, click Report. This logs the incident into Clipboard, so that all associated Staff and Managers can view the details and follow-up where needed.
Top Tip: It is optional to choose a student when reporting an incident, as sometimes incidents occur which involve multiple students, school staff, parents and guardians, or students at another school. You can comment on the incident report to add this information!
Following up on Incidents
Both Return to Play information and Follow up Notes can be added once the report has been submitted. Simply:
Go to Incidents
Click on the specific Incident Report
Write a comment to leave follow-up notes and click the edit pencil icon next to Return to Play to add a date and reason for the student not attending training or games. Then Save.
Once a return to play date has been set, an alert will appear on the student's profile page and on any rolls they are on.

Archiving Incidents:
Do you have any Incidents you need to get rid of? Maybe they are duplicates or test Incidents. Good news! You can now archive Incidents.
This allows you to delete Incidents in a non-permanent way. You can filter the Incidents page to show archived or non-archived Incidents. And you can always un-archive Incidents as well.
Steps to archive an incident:
1. Go to Incidents
2. Click on the specific Incident Report
3. Select archive (bottom right)

What fields are mandatory and uneditable in the Incident Report Template?
Certain fields in the Incident Report Template, such as Activity, Date of Incident, Time of Incident, and Location, cannot be edited. They must be filled out during incident reporting or are automatically populated when linked to a session.
The Activity field has a dropdown menu to specify the activity that the incident occurred during. Therefore it will only appears when the incident is being recorded from outside a session (e.g. on the Incidents page from the side menu).
